Surgical Dentistry

Russian version

The Medical Initiative Medical Center provides planned and emergency surgical dental care, as well as consultations and treatment in the field of dental implantology.

An oral surgeon helps in situations where conservative treatment can no longer preserve the tooth. The doctor also performs tooth-preserving procedures, wisdom tooth extraction, treatment of inflammatory processes, preparation of the oral cavity for prosthetic treatment and other types of surgical dental care.

Tooth extraction is considered only when there are clinical indications — when restoration of the tooth is impossible or when preserving it may maintain inflammation, threaten surrounding tissues or affect the condition of neighboring teeth. The treatment approach is determined by the doctor after examination, diagnostics and assessment of the clinical situation.

chirurgieTooth Extraction in Minsk

Tooth extraction is one of the main areas of surgical dentistry. It may be planned or performed for urgent indications in cases of severe pain, inflammation, trauma or significant tooth destruction.

Before the procedure, the oral surgeon performs an examination, may prescribe X-ray diagnostics if necessary, and determines the optimal extraction method. Anesthesia is selected individually, taking into account the scope of treatment, the patient’s health condition and the clinical situation.

Tooth extraction may be required in the following cases:

  • severe destruction of the crown part of the tooth, making restoration impossible;
  • inflammatory processes in the area of the tooth root;
  • pronounced tooth mobility due to periodontal disease;
  • trauma to the tooth or root;
  • incorrect tooth position;
  • preparation for orthodontic or prosthetic treatment;
  • difficult eruption of wisdom teeth.

The clinic performs tooth extractions of varying complexity, including the removal of severely damaged teeth, tooth roots, and impacted or dystopic wisdom teeth.

Wisdom Tooth Extraction

Wisdom teeth, or third molars, may erupt incorrectly, be positioned at an angle, injure the cheek or affect the neighboring tooth. Their eruption is often accompanied by pain, gum swelling and inflammation of the tissues around the tooth crown.

Wisdom tooth extraction may be recommended in the following cases:

  • difficult eruption;
  • incorrect tooth position;
  • lack of space in the dental arch;
  • inflammation of the tissues around the tooth;
  • tooth destruction that cannot be restored;
  • pathological changes in the surrounding tissues;
  • risk of damage to the neighboring tooth.

The scope and complexity of the procedure depend on the tooth position, root shape and anatomical features of the jaw. To plan treatment, the doctor may prescribe a targeted X-ray or CBCT scan.

chirurgie1Tooth-Preserving Surgical Procedures

The main goal of an oral surgeon is to preserve the patient’s natural tooth whenever possible. If therapeutic treatment is not sufficiently effective, the doctor may consider a tooth-preserving surgical procedure.

Such procedures include:

  • removal of one root of a multi-rooted tooth;
  • removal of part of the tooth together with the affected root;
  • resection of the tooth root apex;
  • treatment of dental trauma, including complete and incomplete tooth dislocations;
  • surgical treatment of cysts and inflammatory lesions in the area of tooth roots.

The possibility of preserving the tooth is determined individually. The doctor assesses the condition of the roots, bone tissue, surrounding tissues and the prognosis of further treatment.

Treatment of Jaw Cysts

A jaw cyst is a pathological cavity in the bone tissue that may form in the area of a tooth root. In the early stages, it may not cause noticeable symptoms and can be detected during an X-ray examination.

As the cyst increases in size, pain, swelling, discomfort when biting, gum changes or destruction of bone tissue may occur.

Surgical treatment of cysts is usually performed on an outpatient basis. After removing the pathological tissues, the doctor may use bone grafting materials to fill the bone defect. If indicated, resorbable membranes may be used to help create conditions for tissue regeneration.

Surgical Preparation for Dental Prosthetics

Before placing removable or fixed prostheses, it may sometimes be necessary to prepare the tissues of the oral cavity. This is required so that the prosthetic construction can be fixed comfortably, does not injure the mucous membrane and distributes the load evenly.

Surgical preparation before prosthetic treatment may include:

  • removal of bony protrusions — exostoses and osteophytes;
  • leveling of the alveolar ridge;
  • correction of soft tissues;
  • treatment of inflammatory processes;
  • preparation of the area for implant placement;
  • restoration of bone volume if clinically indicated.

After the loss of multiple teeth, the jawbone may change, become uneven or undergo atrophy. These features may make it difficult to manufacture and fix prosthetic constructions, so the required amount of preparation is determined by the dentist after diagnostics.

Soft Tissue Surgery in the Oral Cavity

Surgical dentistry also includes the treatment of diseases and changes in the soft tissues of the oral cavity. The doctor may perform correction of the lip or tongue frenulum, treatment of inflammatory processes, removal of benign mucosal lesions and other procedures if indicated.

The removal of lesions is performed after examination and assessment of their nature. If necessary, the removed material is sent for histological examination. This helps clarify the tissue structure and confirm the type of lesion.

Dental Implantation

Dental implantation is a modern method of restoring missing teeth. An implant is placed into the jawbone and, after the osseointegration period, may serve as a support for a crown, bridge or full fixed prosthesis.

Before implantation, diagnostics are performed, including an examination and computed tomography. The doctor evaluates the volume of bone tissue, the condition of the gums, bite and the patient’s general health, and then prepares a personalized treatment plan.

If necessary, tooth extraction, treatment of inflammatory lesions, bone grafting or other surgical preparation may be performed before implantation.

1prfPRF Technology in Surgical Dentistry

What Is PRF?

PRF, or Platelet-Rich Fibrin, is a technology that uses a fibrin clot obtained from the patient’s own venous blood. The material contains fibrin and blood components involved in the body’s natural tissue healing processes.

PRF may be used in surgical dentistry as an additional material after tooth extraction, during dental implantation, bone grafting, sinus lift procedures and periodontal treatment. The decision to use this technology is made by the doctor based on indications and the treatment plan.

When Is PRF Used in Dentistry?

PRF technology may be used:

  • after tooth extraction — to fill the socket;
  • during dental implant placement;
  • during bone grafting;
  • during sinus lift procedures;
  • in surgical treatment of periodontal diseases;
  • in the treatment of gum recession;
  • when it is necessary to create more favorable conditions for tissue healing.

The use of PRF may be especially relevant if the patient has factors that can affect healing processes. The doctor decides whether the procedure is appropriate after consultation, assessment of the patient’s health condition and review of the medical history.

2prfHow Is a PRF Clot Obtained?

Before treatment begins, a small amount of venous blood is taken from the patient — the procedure is similar to a standard blood draw for laboratory testing. The tubes are then placed in a centrifuge.

As a result of centrifugation, a fibrin clot or liquid fraction is obtained, which can be used by the doctor during surgical treatment. The type of material and the method of its application depend on the specific clinical task.

Types of PRF

Different forms of platelet-rich fibrin may be used in dental practice.

A-PRF

A-PRF is a dense fibrin clot. It may be used to fill the socket after tooth extraction, during bone grafting, sinus lift procedures and other procedures related to preserving or restoring bone volume.

I-PRF

I-PRF is a liquid form of fibrin that may be used in the area of soft tissues and in combination with bone substitute materials when clinically indicated.

In dentistry, I-PRF may be used in the treatment of periodontal diseases, gum recession, dental implantation, bone grafting and sinus lift procedures.

4prfPossible Benefits of PRF Technology

Since PRF is obtained from the patient’s own blood, this technology may be included in a comprehensive surgical treatment plan.

Possible benefits of PRF include:

  • use of autologous material — material obtained from the patient’s own blood;
  • support of natural tissue healing processes;
  • possibility of use after tooth extraction and during bone grafting;
  • use in combination with other surgical methods;
  • no need to use donor biomaterial to obtain a PRF clot.

The effect and appropriateness of using PRF depend on the type of procedure, tissue condition, the patient’s general health and compliance with the doctor’s recommendations after treatment.

Frequently Asked Questions About Surgical Dentistry

When Does a Tooth Need to Be Extracted?

Extraction is performed if the tooth cannot be preserved or if its condition may maintain inflammation and threaten surrounding tissues. The decision is made by the oral surgeon after examination and diagnostics.

Is Tooth Extraction Painful?

Tooth extraction is performed under anesthesia. The type of pain relief is selected individually by the doctor. After the procedure, some discomfort may occur, and its intensity depends on the complexity of the extraction and the individual characteristics of the patient.

Does a Wisdom Tooth Need to Be Removed If It Does Not Hurt?

Not always. If the wisdom tooth is properly positioned, participates in chewing and does not cause problems, extraction may not be required. However, in cases of incorrect eruption, risk of inflammation, damage to the neighboring tooth or lack of space, the doctor may recommend removal.

What Is a Tooth-Preserving Surgical Procedure?

This is a type of surgical treatment that allows the doctor to remove a pathological lesion while preserving part or all of the tooth. Such procedures include root apex resection, removal of one root of a multi-rooted tooth and other methods when indicated.

Why Is PRF Used After Tooth Extraction?

PRF is obtained from the patient’s own blood and may be used to fill the socket of an extracted tooth or as part of comprehensive surgical treatment. The appropriateness of PRF use is determined by the doctor based on the clinical situation.

Is Preparation Needed Before Dental Implantation?

Yes. Before dental implantation, a consultation and diagnostics are performed. The doctor evaluates the condition of the teeth, gums, bone tissue and bite. If necessary, treatment of inflammation, tooth extraction or other preparatory procedures may be performed before implant placement.
